Purpose. To analyze results of questionnaire survey of naval medical service specialists on the question of processing of reporting documentation during the cruise.Materials and methods. Questionnaire survey of chief medical officers, analysis of documents, Fisher angular transformation.Results and discussion. Quantitative and qualitative analysis of questionnaires was conducted. The analysis showed that the reporting form shall be simplified and the chief medical officer should be given greater freedom in analyzing various aspects of the medical support of the cruise (the report should be less formalized).Conclusion. Introduction of new reporting forms will allow to evaluate the efficiency of medical component during cruise and to give scientifically based recommendations to military equipment designers, ergonomists, and fleet command on issues of optimization of habitability specifications, implementation of new life-support technologies and lowering the accident rate on naval facilities. ; Цель. Провести анализ результатов анкетирования специалистов медицинской службы ВМФ по вопросу переработки отчетной документации за морской поход.Материалы и методы. Анкетный опрос начальников медицинских служб, анализ документов.Результаты и их обсуждение. Проведен количественный и качественный анализ анкет. Анализ показал, что саму форму отчета необходимо упростить, а начальнику медицинской службы должна быть дана большая свобода в анализе различных аспектов медицинского обеспечения похода (отчет должен стать менее формализованным).Заключение. Внедрение обновленных форм отчетности позволит не только оценить эффективность медицинской составляющей в плавании, но и дать научно-обоснованные рекомендации проектантам военной техники, эргономистам, командному составу флотов по вопросам оптимизации технических характеристик обитаемости, внедрению новых технологий жизнеобеспечения и снижения аварийности на объектах ВМФ.
The article suggests a theoretical framework to analyze the impact of protest publics on the mechanism of political changes. An analysis of protests in Egypt and Turkey shows the non-linear nature of the political changes. It also highlights different roles that the protest publics could play in the mechanism of political changes: depending on the prevailing conditions and the actions of other actors, protest publics can build coalitions with other actors and provide democratic changes ("triggers" of democratic development) or protest publics can provide democratic alternative by developing democratic practices of civil participation. This trend is marginal, though, comparing with the dominant tendency towards strengthening of authoritarian regimes ("democratic innovators in authoritarian environment)." Authors raise further research questions of modeling the mechanisms of political changes in the conditions of the growing wave of democratization – in embedded democracies, as well as in consolidated democracies, in hybrid and authoritarian regimes, – on the one hand, and the crisis of traditional democratic institutions – representative democracy, political parties – on the other. ; Данная статья предлагает теоретическую базу для анализа влияния протестной публики на механизмы политических изменений. Анализ протестов в Египте и Турции показывает нелинейный характер политических изменений. Он также выдвигает на первый план различные позиции, которые протестная публика в этих странах может потенциально занимать в механизме политических изменений: в зависимости от условий и действий других субъектов, протестная публика может создавать коалиции с другими участниками и обеспечить демократические перемены (т.е. являться «триггером» демократического развития) или же обеспечивать демократическую альтернативу путем развития демократической практики гражданского участия. Хотя эта тенденция и незначительная по сравнению с доминирующей тенденцией к усилению авторитарных режимов («демократических новаторов в авторитарной среде»). Авторы поднимают дальнейшие исследовательские вопросы моделирования механизмов политических изменений в условиях растущей волны демократизации – в имитационных демократических режимах, а также в консолидированных демократиях, в гибридных и авторитарных режимах, – с одной стороны, и кризиса традиционных демократических институтов – представительской демократии, – с другой.
The article presents the background of healthcare system computerization development starting from the time when preconditions for IT systems implementation appeared up to the present moment. It outlines the problems that emerged in the course of implementing the projects aimed at computerizing the RF Healthcare System and providing a high-technology support for it; it features also the indicators related to financial investments into the healthcare information systems. Based on the reports of the authorized agencies the project implementation efficiency is analyzed and solution options for the existing problems are shown. The article reveals development prospects for IT implementation in healthcare.Objective. Study of IT systems development and incorporation into the practical healthcare structure of the Russian Federation based on the freely available data. Laws and regulations, including reports in healthcare, publications of Russian and foreign lead researchers in economics and medical administration, analytical reviews of consulting companies such as RBC, Price Waterhouse Coopers (London, UK), C-News Analytics (Russia, Moscow), and Vademecum business magazine (Russia, Moscow).Results. The healthcare computerization process in Russia is slower than it is set out in the Governmental plans. The most common and significant problems include the following: lack of funding for the computerization process in general; irrational distribution of resources within the projects and selection of a costly computerization strategy from the beginning; corrupt practices; conflicts between customers with the contractors because of which many projects were not implemented; management problems; a clash between the current legislation to modern technologies and trends, lack of efforts aimed at implementation of healthcare information systems on the part of the regional authorities.Conclusion. To make the healthcare computerization more successful it is required to awaken a commercial interest in the private IT companies to attract additional investment. Personal responsibility of the officials in charge of the computerization should become more stringent in case of a delay in a project or a project failure, the government procurement system should be improved in order to attract the most qualified integrators, the interaction between healthcare managers at various levels with the integrators should be facilitated through reducing bureaucratic «paper» load. At the state level it is necessary to focus on the operational improvement of the RF legislation to ensure the progressive development and implementation of information technologies. ; В статье рассматривается история развития процесса информатизации в системе здравоохранения Российской Федерации (РФ) с момента формирования предпосылок к внедрению информационных систем до настоящего времени. Обозначен ряд проблем, которые возникли в процессе реализации проектов, направленных на информатизацию и высокотехнологичную поддержку системы здравоохранения РФ, в том числе финансирование таких проектов.Цель работы заключается в исследовании вопросов развития и внедрения информационных систем в структуру практического здравоохранения РФ по данным, размещенным в свободном доступе. Проанализированы нормативно-правовые акты, в том числе отчеты в сфере здравоохранения, публикации ведущих отечественных и зарубежных исследователей в сфере экономики и управления здравоохранением, аналитические обзоры консалтинговых компаний, таких как «РосБизнесКонсалтинг», Price Waterhouse Coopers (г. Лондон, Великобритания), C-News Analytics (г. Москва, Россия) и делового издания Vademecum (г. Москва, Россия).Результаты. Процесс информатизации здравоохранения в России идет медленнее, чем это обозначено в правительственных планах. Среди наиболее распространенных и существенных проблем можно выделить следующие: недостаточность финансирования всего процесса информатизации в целом; нерациональное распределение средств бюджета внутри проектов и выбор изначально затратной стратегии информатизации; коррупционные схемы; конфликты заказчиков с исполнителями, из-за которых многие проекты так и остались невыполненными; проблемы менеджмента; несоответствие действующей законодательной базы современным технологиям и тенденциям; недостаточные усилия региональных властей по внедрению медицинских информационных систем.Заключение. Для более успешной информатизации здравоохранения необходимо стимулирование коммерческого интереса частных IT-компаний для привлечения дополнительных инвестиций. Стоит ужесточить персональную ответственность должностных лиц, курирующих разработки в области информатизации, в случае срыва сроков и невыполнения проекта; усовершенствовать системы госзаказа для привлечения самых высококвалифицированных интеграторов; облегчить процесс взаимодействия менеджеров здравоохранения различных уровней с интеграторами путем снижения бюрократической бумажной нагрузки. На государственном уровне необходимо отдельно уделить внимание оперативному совершенствованию законодательства РФ, которое обеспечивало бы прогрессивное развитие и внедрение информационных технологий в систему здравоохранения.
